Minnesota Department of Public Safety requires that this form be completed before any instructions are given.
Terms of Sales:
* Students have 6 months to complete 30 classroom hours (10 classes) from the day student starts classroom or from student’s 15th birthday if you start when you’re 14, or remaining lessons will be forfeited and no refund will be given.
* Students have 6 months to complete 6 hours (3 lessons) of behind the wheel from last day of classroom or until their 16th birthday, whichever is longer, or remaining lessons will be forfeited and no refund will be given.
* Behind the wheel appointments that are canceled with less than 48 hours notice (including no shows and no permit) will be charged $80, no exceptions.
* Behind the wheel lessons are subject to a fuel surcharge of $10.00 per lesson, payable to the instructor.
* Other students may be in the vehicle during behind the wheel lessons.
* This agreement constitutes the entire contract between the school and student/parent. No verbal statements or promises will be recognized.
